Licensed Master's Social Worker (LMSW)
1 - 3 years of experience required
The Social Worker works collaboratively with the Geriatrician, patient and fa! mily to develop a patient-centered health plan that is based on a comprehensive assessment of goals, cognition, function, support systems, and caregiver burden. The Social Worker assists with adjustment to new life situations and initiates referrals for community resources as appropriate.
Masters Degree in Social Work from an accredited program required. State of Michigan license, LMSW required. Certificate in Gerontology preferred. 2 years as an MSW Social Worker in Gerontology.
