Provides medical care services under the supervision of an approved physician or properly designated alternative physician in the ACE (Acute Care for Elderly) ! Unit.
UNIT DESCRIPTION:
- 16 Bed unit housed on the 10th floor of St. Joseph Mercy Hospital in Ann Arbor and managed by a joint venture with IHA, St. Joseph Mercy Hospital, and the University of Michigan.
- The ACE Unit is dedicated to providing acute care for the elderly by using a multidisciplinary approach and evidence based guidelines to address the needs of this unique patient population.
- The Unit will likely expand to 32 beds in the near future.
- Qualified AHPâs will have ANP training in an accredited program. Further, they will have additional training and certification in Geriatric Care.
- The ANP will be employed by IHA and work in conjunction with the Geriatricians rounding in the unit with the essential functions and responsibilities outlined below.

- Provides quality patient care by considering the age specific, developmental and cultural needs through competent clinical practice. Demonstrates unit/area competencies.
- Behaves in accordance with the Mission, Vision and Values of Saint Joseph Mercy Health System.
- Completes patient work-up including patient history, physical exam, order appropriate laboratory tests, x-ray studies, and any other diagnostic studies or procedures needed.
- Interprets test results and physical findings.
- Writes initial and ongoing orders and medications and discusses plan of care daily with the attending physician.
- By law may give verbal orders, which are subsequently co-signed by the attending physician to registered nurses adhering to medical staff policies regarding the use of verbal orders.
- Sends consultations to physicians, allied health professionals, health care agencies, and nursing homes.
- Prescribes physical, occupa! tional, and speech therapies. This requires a co-signature in the chart within 24 hours.
- Makes daily hospital work rounds including pre- and post-operative and charts progress notes as required or as necessary.
- Dictates discharge summaries, chart rounds, and staffing conference summaries.
- Prescribes outpatient medications and therapies (must list the attending physician's name or have co-signed.
- Performs diagnostic or therapeutic procedures as needed.
- Provides education to patients and their families regarding health status and health maintenance.
- Evaluates and coordinates a plan of care in conjunction with attending physician and other team members.
- Participates in-patient care conferences and peer quality review with attending and other staff members to assure quality patient care.
- Participates in patient care hand-off processes.
- Works in a collaborative and collegial manner with all members ! of the health care team
- Assigns a code status after consultati! on with the attending physician (to be co-signed within 24 hours.
- Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution.
- Maintains the confidentiality of information acquired pertaining to patient, physicians, employees, and visitors to St. Joseph Mercy Hospital and University of Michigan ACE Unit. Discusses patient and hospital information only among appropriate personnel in appropriately private places
- Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.
