With continued growth and high patient demand Henry Ford Health System's Department of Family Medicine, Division of Geriatric Medicine is seeking a board eligible/certified Family Medicine or Internal Medicine physician with Geriatrics experience to practice with Henry Ford's Home Visiting Physicians Program. This position is employed by the Henry Ford Medical Group.

Responsibilit! ies
The Henry Ford Medical Group is looking for a full time Geriatrician to care for patients in the metro-Detroit area. Candidates must be board certified/eligible in Family Medicine or Internal Medicine. Geriatric Fellowship training is preferred, Geriatric experience required.
Henry Ford Visiting Physicians (HVP) provides continuity of care in the home as well as geriatric health care to individuals with limited access and mobility to traditional primary care. In business since 2007, HVP specializes in geriatric care, serving senior and homebound patients throughout Southeastern Michigan's Tri-County area in order to meet a variety of helath and geriatric care needs.
Our visiting physicians are dedicated, compassionate individuals who are highly trained in the diagnosis and treatment of geriatric and homebound patients. Advancements in medical technology allow our physicians the ability to provide comprehensive in-home medical care to their pat! ients.
Visiting physicians are prepared to provide the same se! rvices that you would receive in a traditional primary care office setting. This integrated health care model includes physical examinations, disease management, medication management, laboratory testing, and portable diagnostics. We also specialize in home care coordination within the Henry Ford Health System and other agencies.
The care team includes physicians, nurse practitioners, registered nurses, social workers, and medical assistants. The team coordinates all of the in-home care including home health services, x-rays, ultrasounds, durable medical equipment, laboratory services, and specialist referrals.
Qualifications
- Must be board certified or eligible in Family Medicine or Internal Medicine
- Must have Geriatrics experience
- Geriatrics Fellowship training preferred
- Residency/Fellowship training by an ACGME or AOA accredited program is required
